Habitat-Ready

Habitat-Ready shows which support is needed at home, who takes responsibility and which handovers remain open. Goals, capacities, burdens and available services form a care constellation that changes as new information arrives.

Connect perspectives

People receiving care, relatives and professionals contribute different experiences. Habitat-Ready distinguishes personal goals, observed abilities, assessments and firm commitments. It shows whether support has been requested, promised or actually made available, and which tasks relatives willingly accept.

Follow handovers

A transition consists of concrete agreements: delivering an aid, starting a service, reviewing medication or arranging follow-up. Each task has an owner and a reviewable status. Feedback after discharge shows which agreements have become part of everyday life.

Consider next steps

New observations update the case picture. The model presents options with their assumptions and calculations for discussion: clarifying roles, practising an everyday task together or arranging relief. Professional judgement and participants’ goals guide the next step and the timing of review.
